Plan Goal
Prevention through Education is the best plan in regard to diabetes in San Miguel County.
Situation Statement
Diabetes continues to be a major health problem for both adults and youth. An increase in calls for private consultation and advise on diabetes has made it necessary to rethink the actual way San Miguel County handles these clientele, A support group has been formed and nutrition education has began, and will continue, the goal is to provide group training and support to help people understand the importance of diabetic education. A Mini series diabetes classes (5 sessions) have been implemented and the outcomes have been very positive. One to one contact continues with those who find it necessary to keep closer contact with the Home Economist and support group leader.
Target Audience and Actions
Target group presently includes the adult population. Will continue to provide one to one support along with support group leader programming. Will host Kitchen Creation programing to fulfill the needs of the county clientele. An expansion into the diabetic youth group is in the planning state at present. In the fall we expect to address the schools with a presentation on the importance of proper diet, and health care of those with the illness.
Short-Term Objectives
80% Clientele will gain diabetes knowledge, adopt skill learned and aspire to become more aware of how to select the most appropriate foods in a diabetic diet.
80% of the clientele reached will be able to identify the right foods for diabetic.
80% of clientele will increase physical activity to improve personal health.
Medium-Term Objectives
Diabetic Clietele will practice and adopt behavioral changes after learning the diabetic 50/50 concept.
70% preception of special diabetic diets will be resolved.
Long-Term Objectives
Over 50% of clientele will maintain the practices learned from the 50/50 training.
50% of these clientele will maintain better diabetic levels.
50% will achieve desired weights.
Evaluation Plan
Diabetic knowledge and behavior surveys will be used to measure short and medium term objectives.
One to one contact with individuals will assess behavioral changes.
